Screening Test for School Age Children
(5 years of age and older)

Directions

Read each question on the checklist carefully. Check the box next to each warning sign that applies to your child. At the end of the survey, click on the calculate button to obtain a score and recommendations.

Does your child demonstrate difficulty organizing words into sentences after age 5 (e.g., child uses 'Who name?" instead of "What's your name?"; child uses "He go to him school." instead of "He is going to his school.")?
Does your child demonstrate difficulty understanding speech (e.g., stories, conversations, television programs)?
Has your child demonstrated prolonged or repeated ear infections in the first five years of life?
Does your child shift from task to task and/or have difficulty waiting for his turn (e.g., child appears to be impatient and/or interrupts)?
Does your child demonstrate problems staying seated or standing in one place (e.g., child has a short attention span, is impulsive and/or seems overactive)?
Does your child demonstrate difficulty following directions (e.g., "Put your shoes away and hang up your coat.")?
Does your child demonstrate difficulty recalling and retelling events and stories (e.g., story is out of order, or is missing important information)?
Does your child have difficulty telling and understanding jokes (e.g., child does not laugh at appropriate times)?
Does your child demonstrate speech that includes a lot of hesitations (e.g., "Um....um", "You know") and have difficulty getting to the point?
Does your child demonstrate difficulty staying on topic in conversations (e.g., when you are talking about Pokemon, the child starts taking about hockey)?
Does your child demonstrate difficulty making and keeping friends (e.g., other children avoid playing with him/her at recess)?
Does your child have difficulty following the rules and activities of the classroom (e.g., teacher reports that the child is disruptive in the classroom)?
Does your child demonstrate difficulty reading at his/her age level?
Does your child demonstrate difficulty organizing ideas into written form (e.g. teacher reports that journal writing is not at the level of other classmates)?
Does your child demonstrate a lot more difficulty in grade 3 than in previous grades.
