Information We Collect We collect and use information that is necessary to run our business, to provide speech-language assessment, therapy, and / or academic assessment and tutoring, and to keep you informed about all our services and programs. We Collect Information From London Speech & Language Centre Case History forms, from Speech-Language Pathologists involved in your care, treating Health care practitioner and occasionally other professionals involved in care (e.g. lawyers, insurance, case managers). This is for the purpose of proper assessment and to provide you with appropriate services. How We Use Information About You We use client information to provide appropriate assessment information, programming, goals, treatment strategies, family caregiver support with your signed permission and on occasion information may be used for public education purposes. Information Disclosure We share
information about our clients with London Speech and Language Centre staff
directly involved in your care. Your Awareness and Permission Before we
share or obtain any personal information with external organizations and
professionals, we must obtain your signature on London Speech and Language
Centre’s “Authorization to Obtain & Release Information”
forms. Your authorization for any communication can be terminated at any time by submitting a written request to London Speech and Language Centre’s Information Officer, Chris Knight. We maintain physical and electronic safeguards to protect client information.
